Adriamycin Extravasation Protocol
If there is doxorubicin extravasation act immediately
- Do not remove IV catheter when extravasation occurs.
- Prepare for safe handling of chemotherapy agents that may be extracted from the patient. These include: chemotherapy gloves, absorbent pads under the leg, biohazard bag and goggles.
- Use a syringe to remove as much drug as possible from the IV catheter tubing and tissue. **DO NOT FLUSH CATHETER**
- If there is a “bleb” use a 22 gauge needle attached to a syringe and aspirate as much of the drug as possible. Then remove IV catheter.
- Place light wrap over the extravasation site.
- Apply cold compress to the extravasation site for ~30 minutes, repeat in 1-2 hours.
- Advise owners to use ice cold packs or cold compresses every 6 hours on the extravasation site for the first 24 hours if possible.
- Use with caution in patients with renal or hepatic insufficiency
- Place a new patent IV catheter in another vessel
- Reconstitute vial contents and dilute before use
- Dosage ratio of dexrazoxane to doxorubicin is 10:1 (i.e. 300mg/m2 dexrazoxane to 30mg/m2 doxorubicin)
- Dilute further with a weight appropriate amount of 0.9%NaCl
- Administer dexrazoxane by intravenous infusion over 15 minutes
- Administer immediately after extravasation (at least within 2 hours) and 24 hours and 48 hours after extravasation
- Monitor extravasation site weekly or as often as indicated
